the age of 18. If you are
18 years of age or older,
you'll fill out our Release
of Liability or French
Release of Liability Form when you get to the gym.
The .pdf download file includes 2 pages. The first is a covering letter to parents, explaining what the children or teens will be doing at The Boiler Room. The next page is the Informed Consent Agreement (Consent Form). Please print it from your computer and then make sure it is filled out in pen and signed by your parent or legal guardian. Don't forget to bring it with you to the gym on your first visit!
